Resident Conversion

 

  • Medical Doctors or Osteopathic Doctors who currently hold an active Resident Certificate with the South Dakota Board of Medical and Osteopathic Examiners should use the application below to convert their Resident Certificate to a Physician License.  The Board will contact you about additional requirements after we have reviewed your application.
  • If you do not have a South Dakota Resident Certificate issued by this Board, do not use this form.
